What's On Your Mind?
This form submission page is provided as a service of FODHS to help you quickly (and anonymously if you prefer) report a problem you see in your city. 

Your submission is automatically forwarded to the city code department or graffiti control department at city hall. Only if you would like a reply by phone, you will need to leave a telephone number. Your email is required for the form to function. Your email will NOT be used to contact you or to send you an email unless you specifically ask for a reply. Emails addresses are not saved - we don't have the time or the desire to do so.
